We are providing this consent form to inform you and to request your permission for your child’s photo/image and personally identifiable information (name and school) to be published in print and social media, including the Foundation’s website.

As you may be aware, there are potential dangers associated with the posting of personally identifiable information on a website. Global access to the Internet does not allow us to control who may access such information. These dangers have always existed; however, we do want to celebrate your child and his/her service to the community.

If you, as the parent or guardian, wish to rescind this agreement, you may do so at any time by contacting the Fair Foundation in writing and such rescission will take place upon receipt of your request.
